Bursitis (Trochanteric bursitis)

Inflammation of the bursa or bursae within the hip joint can cause pain in one or both hips. Bursitis typically presents as pain on the outer side of the hip and may feel worse when lying on the affected side due to increased pressure on the inflamed bursa.

Trochanteric bursitis affects an estimated 10–25% of adults at some point in their lifetime. It is more common in women, with a female to male ratio of approximately 4:1. While it can occur at any age, peak incidence is seen between 40 and 60 years.

Bursae can become inflamed due to repetitive movements such as those involved in sport, athletics, or physically demanding occupations. Injury is a common contributing factor. In rarer cases, infection, including conditions such as osteomyelitis or septic arthritis, may be the underlying cause.

Tendonopathy

Hip pain that is accompanied by swelling and tenderness is most often caused by tendonopathy ( previously called Tendonitis) . Tendon pain can result in pain due to inflammation of the Gluteal tendon. This can be due to any of the reasons mentioned above or due to overload or impact.

The role of tendons in our bodies is to connect muscle to bone. Activities that repeatedly strain the hip tendons can lead to agonising pain. Tendonopathy of the hip can be experienced by sporty and not sporty populations and the treatment and recovery processes can vary depending on each specific case.

Hip Pain and Osteoarthritis

Often called ‘wear and tear’ and associated with age due to its degenerative nature, osteoarthritis is a form of arthritis that affects the cartilage between joints such as the hip, knee, ankle, wrists and shoulders. In most instances, osteoarthritis is diagnosed using X-rays or MRI scans and a physical examination.

Though there is no cure, if you have received a diagnosis of osteoarthritis, there are several things that you can do to help reduce your hip pain.

Firstly, it is very important to be aware that the diagnosis of osteoarthritic change in the hip or knee joint does not immediately imply that you need a hip or knee replacement.

The next step is to select suitable exercises that help support the joint and build muscles to slow further unfavourable joint loading.

Hip Labral Tear (mild to moderate cases)

The hip joint contains an additional layer of semi-fibrous cartilage known as the labrum. This structure deepens the hip socket and helps promote stability of the hip joint during movement.

Like any tissue in the body, the labrum can become damaged. This may occur due to poor movement mechanics, muscle imbalances, or direct injury. In mild to moderate cases, a hip labral tear commonly causes a deep, aching pain in the hip, sometimes accompanied by sharper pains during certain movements.

Symptoms may worsen with activity, and pain can spread into the groin, outer hip, or thigh, particularly after prolonged walking, running, or exercise.

Hip Impingement (Femoroacetabular Impingement – FAI)

Femoroacetabular impingement occurs when structures within the hip joint become pinched or caught during specific movements. This is commonly experienced by active individuals and young adults. Symptoms often begin as a pinching or catching sensation in the hip and can progress to persistent pain over time.

As the condition develops, hip pain is frequently accompanied by stiffness and a reduced range of movement. If left unaddressed, symptoms may worsen to the point where the individual is unable to continue participating in sport or physical activity.

FAI typically responds well to a structured corrective rehabilitation programme. This focuses on addressing muscular imbalances around the hip joint and improving joint mobility through targeted exercises. In more complex cases that do not fully resolve after a consistent three to four months of rehabilitation and treatment, surgical intervention may be considered.

Muscle Strains (e.g., hip flexor or groin strain)

Possibly the most common hip pain injury that we provide hip treatment for in Cork is muscle strain. Muscle strain can happen to anyone and can be caused by simply overuse, a sudden forceful injury or even an incorrect sports stretch.

Mild hip muscle strain is easy to treat, and you should recover in a few weeks with the help of some treatment and rehabilitation. More severe muscle strain in the hips requires more extensive rehab and return to sport planning.
