The Five Leading Causes of Neck Pain
There’s a reason people describe annoying situations as ‘a pain in the neck’; it’s because neck pain is one of the most irritating conditions and is hard to ignore. It is very difficult to avoid aggravating the neck when pain is present. Turning side to side, driving the car, putting on clothes, and even sleeping can all be a challenge when neck pain is in full flow and pain medication barely takes the edge off.
The good news is that physiotherapy can help reduce pain through a range of skills and treatment modalities, including soft tissue and fascial work, dry needling, joint mobilisations, laser therapy, also known as photobiomodulation, shockwave therapy, and postural corrective exercises.

Muscle strain and tension
Neck muscle strain and tension have been common complaints for many years, but they have become increasingly prevalent as lifestyles have grown more sedentary. Reduced levels of physical activity have led to many people becoming physically deconditioned, meaning less fit and less resilient to everyday postural demands. This is particularly evident in desk-based workers, where prolonged screen use and sustained poor posture commonly contribute to neck pain.
Forward head posture is a key factor. Research by Hansraj (2014) demonstrated that as the head moves forward from a neutral position, the mechanical load placed on the cervical spine increases significantly due to leverage and gravity. In a neutral position, the head places a load of approximately 10–12 lbs on the neck. At 15 degrees of forward flexion, this load increases to around 27 lbs, rising to approximately 40 lbs at 30 degrees, 49 lbs at 45 degrees, and up to 60 lbs at 60 degrees of forward flexion.
The good news is that neck muscle strain and tension respond very well to sensible, evidence-based physiotherapy advice and treatment, with a focus on improving posture, restoring movement, and rebuilding strength and resilience in the neck and upper body.

Joint stiffness
Joint stiffness is typically associated with age-related changes, including arthritis, and can cause neck pain as well as the knees, shoulders, ankles and wrists.
Joint stiffness can also be caused by a lack of movement. In many cases, this can be a “use it or lose” it situation. The more we move our bodies, the better our bodies move. When we spend more time sitting than moving, joint stiffness creeps in, which can lead to reduced mobility.

Disc irritation or herniation
In some cases, a bulging or degenerating disc may irritate nearby nerves, leading to pain and discomfort.
Our spines are made up of a series of small bones called vertebrae, which reach from our tailbone up to the base of the skull. Between each vertebra is a cushion of sorts. These are the discs. Disc irritation is usually caused by inflammation of one or more discs.
Herniation differs from disc irritation. A herniated neck disc bulges out between the vertebrae. Here, it becomes trapped and irritated. A key indicator of a herniated neck disc is pain that is accompanied by numbness or tingling in the upper limbs.

Whiplash injuries
Sudden jolts caused by car accidents or sports can strain the soft tissues in the neck, causing painful injuries known as whiplash. If you have experienced neck pain following an accident in which your head and neck were forcibly propelled forward and back, you may have whiplash and should seek professional help immediately.
Other indicators of whiplash include headaches, back pain and feeling dizzy as well as pain and limited movement.

Sleeping position and pillow choice
How we sleep is of great importance, but is regularly overlooked. Having the wrong pillow (or even too few or too many pillows) can place unnecessary stress and strain on the neck as you sleep, resulting in a stiff neck in the morning.
